Faribault Area Hospice Foundation financial assistance is available to hospice enrolled patients connected to District One Hospital or its service area with either financial hardship or those in hospice who would benefit from additional complementary therapies such as massage therapy, music therapy, aromatherapy, and acupuncture. The District One Hospital service area has traditionally been the following zip codes: 55021 (Faribault), 55946 (Kenyon), 55053 (Nerstrand), 55087 (Warsaw), and 55052 (Morristown). The funding is provided through generous community donations, and seeks to help bridge gaps for hospice patients, providing some financial support for things such as gas and groceries, electric bill, limited hospice house stay or in-home care.
Please provide us with a brief summary of why the patient is in need of financial assistance (and what type is requested) or if coverage for additional complementary therapy/therapies is desired. The application will be reviewed by a committee and you should be contacted by email within 72 hours regarding the committee's decision.
